Back to School

Welcome back to school! 

Required Health Office forms can be found under the "Health Forms" tab on our webpage.

  • Every student should come to school with a completed Emergency Sheet each year. Please take a moment to download and complete the form and send it in with your child on the first day of school. Forms can also be faxed or emailed to the Health Office. 

  • Students in Grades PreK or K, 1, 3 and 5 will need current physical. Please contact your child's doctor to schedule an appointment or request that a copy is forwarded to our office.

  • If your child requires medication to be kept in the Health Office and administered by the School Nurse, Mrs. Rydell, the Medication in School form must be completed. It needs to be signed by both the parent and health care provider.

  • Provide a change of clothing for students in ALL grade levels. There are many reasons why changing clothes would be necessary. The Health Office has a very limited amount of spare clothing and your child will likely be more comfortable wearing their own clothes.
